The purpose of this thesis project was to determine the metabolic interactions between the herbal medication, St. John's Wort, and the anesthetic induction drug, propofol. St. John's wort and propofol are both partially metabolized by the cytochrome P450 system. Specifically, hypericin and hyperforin, two active ingredients of St. John's wort, have been shown to induce the cytochrome P450 metabolic system. The effect of hypericin and hyperforin on the metabolism of propofol by the cytochrome P450 pathway was measured in vivo with liver microsomes using a high pressure liquid chromatographic method (HPLC) and UV detection. Chromatographic conditions included: mobile phase of 6:4 acetonitrile and 20mM phosphate buffer (pH 4.52), flow rate of 0.8ml/min, and UV detection of 220nm. Propofol, potassium phosphate buffer, NADPH solution and liver microsomes were added to a test tube. NADPH was used to initiate the reaction, ethyl acetate was then added to stop the reaction, and 2-tert-Butyl-6-methyphenol was added as the internal standard. Time course and dose response curves were established for propofol and varying doses of hypericin and hyperforin were then separately introduced. This research project found that hypericin and hyperforin had no effect on the metabolism of propofol via the cytochrome P450 system.
